Deadlines:
September 1 and March 1 (postmarked or delivered).
Notification of results within 2 months.
Priority for funding will be given to projects that:
- Build ~ collaborative relationships and partnerships,
- Engage ~ community members, clients and consumers in planning, implementation and evaluation,
- Develop ~ and strengthen community leadership,
- Leverage ~ matching funding or resources for the organization.

Regarding Agreement to Grant Terms: If sending by email, please scan the signed agreement and include in your email. If you do not have the capability to scan the agreement, please send in a signed copy by Postal Mail.
Guidelines for submitting application by Postal Mail:
1. Submit:
- Grant Application Form (12 copies),
- Attachments (See Application Form) - four pages maximum – (12 copies),
- Project Description, including:
- Need for the project;
- Expected outcomes, including who and how many will benefit;
- How clients or consumers have been engaged in various stages of project development;
- Timeline;
- Community partners;
- Relation of this project to other programs or projects in the community;
- Up to 3 letters of support (in addition to the 4 page maximum);
- Project Budget--showing total project costs and how grant funds will be spent;
- Description of Organization (brief), incl. scope of programs, who and how many you serve;
- One copy of the applicant organization’s letter of tax exemption (IRS 501(c)3 letter);
- One original signed Agreement to Grant Terms (keep a copy for your records).
